This license is required for the sale of cigarettes, tobacco, and vape products over the counter or from a vending machine.
Apply
You can apply online using this self-service portal. You can upload any needed documents and process payments online. The following documents are required:
- CTV-100 Cigarette, Tobacco, and Electronic Vaping Device Retail License Application
- CTV-101 Cigarette, Tobacco, and Electronic Vaping Device License – Individual Questionnaire
- CTV-102 Cigarette, Tobacco, and Electronic Vaping Device Appointment of Agent
- WI Tax Registration/Seller’s Permit
The licensing period for this license is July 1 – June 30. Renewal applications must be submitted online through the self-service portal.

Fee
The annual license fee is $100, plus a $15 background check fee for the agent.
